FOR ALL VISITS: If you are sick, have a fever, or have been exposed to COVID-19 or other illness at the time of your appointment, we kindly ask you to notify us at (716) 626-0093 and reschedule. We appreciate your assistance in keeping our team, our clinicians, and other patients healthy!

Preparing for your first appointment

  • Dress in clothing you can move around in. Your body will be examined during your visit, and if it cannot be examined you may be asked to put on a gown.
  • Bring CDs of all recent imaging studies that have been performed (last 2-3 years). We want to look at the images and your referring doctor can’t send these.
  • Bring a current list of your prescription medications, over-the-counter medications and supplements.
  • Plan to arrive at least 20-30 minutes before your scheduled appointment time to allow for registration and to complete paperwork.
  • Bring your insurance card and picture ID. If you are being seen under workers’ comp or no-fault, please bring all case numbers and documentation as well.
  • Verify the correct location of your appointment. We have convenient locations in Buffalo, Williamsville, and Orchard Park.
  • A co-pay will be collected at the time of your appointment if required by your insurance.

Preparing for your revisit

• You may be following up with the doctor or the physician assistant. Both providers work together to provide you with excellent musculoskeletal care.
• Bring your insurance card and any updated contact information.
• Bring CDs of all imaging studies that have been ordered. The imaging facility has been asked to provide you with a CD, please ensure you get it.
• Bring your completed pain journal if given one after your procedure.
• There is a fee for completion of forms.
• Verify the correct location of your appointment. We have convenient locations in Buffalo, Williamsville, and Orchard Park.
• Plan to arrive 10 minutes prior to your appointment time to allow for registration and to update information as needed.
• A co-pay will be collected at the time of your appointment if required by your insurance.

Preparing for your spinal injection or procedure

  • Wear loose fitting clothing that can be moved. You may be asked to put on a gown.
  • Come in with clean skin, take a shower or bath before the procedure if possible.
  • If you are on warfarin or coumadin, know your last INR level. You may have been given a lab order to have this completed before your injection.
  • Notify the clinic BEFORE your scheduled procedure if any of the following apply:
    • You are sick, have a fever, or are on an antibiotic
    • You have had any type of vaccine (flu shot, shingles shot, COVID-19 booster, etc.) in the last 14 days
    • FAILURE TO DO SO MAY RESULT IN YOUR PROCEDURE BEING CANCELLED OR POSTPONED.
  • Verify the location of your appointment. Please note that spinal procedures are ONLY performed in our Williamsville location (100 College Parkway, Suite 100).
  • For spinal procedures, make sure you have a driver unless otherwise instructed.
  • You do NOT have to be fasting for procedures unless otherwise instructed.
  • Take all your normal medications as directed prior to your procedure.

Preparing for your EMG/NCS test

  • Wear loose fitting clothes, preferably a t-shirt or tank top for upper extremity studies and shorts for lower extremity studies. You may be asked to put on a gown.
  • No lotions, oils, Vaseline, etc. to your skin before your test.
  • Remove rings, bracelets, watches and other jewelry before your test.
  • Notify clinic before your test if you have an infection of the skin in the extremity being tested.
