Preparation for First Physical Therapist Visit:

Arrival

Please prepare to be in the clinic 20 minutes before your scheduled time. This is to ensure that your insurance is verified and all relevant forms are completed. Out-of-office referrals (patients not being seen by a Buffalo Spine physician), please bring your physical therapy script from your physician. Additionally, please bring any relevant medical information (such as X-rays, MRI impressions, or medication lists)

Attire

Comfortable clothes should be worn during physical therapy visits. For example, if you are being evaluated for a hip injury, do not wear jeans that can restrict movements. If you are coming from work, there are bathrooms in the lobby area of the main building to change. If this is the case, please allow yourself time to do so.

Evaluation

You will have a thorough evaluation that will involve a relevant history and a physical examination. This evaluation should take roughly 30 minutes. It can be helpful to your therapist to think of 3 main issues that your pain or weakness is causing to create a goal for your treatment plan.

Creating a Treatment Plan

After your evaluation, your physical therapist will discuss treatment of your injury. You will collaborate on number of visits per week, (typically 1-2x/week), prognosis, as well as what exercises should be done at home. Being compliant with a home exercise program is imperative to prepare for your next visit.

Cancellation Policy

Our staff is dedicated to helping you achieve your goals; however, we need your assistance. Physical therapy and your recovery are only as effective as your willingness to attend treatment consistently. We reserve time to evaluate and treat your problems. If you cancel or no-show, the policy of our clinic is to discharge patients from care who cancel 3 times or no show for an appointment 2 times. We will notify your physician of this action

Please Note: If you do not show for your appointment, cancel within 24 hours of your appointment or you are more than 15 minutes late, you will be subject to a $25.00 fine.
